چکیده

CNS: central nervous system INTRODUCTION Cryptococcosis is an invasive fungal infection most often caused by the encapsulated yeasts Cryptococcus neoformans and Cryptococcus gattii. C neoformans is the major pathogenic member of the genus, thought to account for approximately 80% of isolates worldwide, and is found in pigeon droppings, soil, and rotting vegetation. The incidence in the United States is approximately 5 cases per 100,000. However, the global burden is much more significant, with more than 1 million new cases and more than 600,000 deaths a year. Most patients with cryptococcosis are immunocompromised because of HIV infection, immunosuppressive therapy, or malignancy. Although cryptococcal infections begin in the lungs via inhalation of the basidiospore, the classical clinical manifestation is meningoencephalitis. Nonmeningeal, nonpulmonary symptoms are less common and generally reflect disseminated disease. Cutaneous manifestations are seen in approximately 15% of patients with systemic cryptococcosis and can have a variety of presentations, including papules, plaques, and ulcers. Here we present an uncommon case of cutaneous cryptococcosis presenting as cellulitis in a heart transplant recipient.

Disseminated cryptococcosis presenting initially as lower limb cellulitis in a renal transplant recipient – a case report

BACKGROUND Cellulitis is an unusual presentation of disseminated cryptococcosis, a serious infection seen predominantly in immunocompromised hosts. Disseminated cryptococcosis carries significant morbidity for transplant recipients, especially of the pulmonary and central nervous systems, and carries a high mortality risk. CASE PRESENTATION We report a 59-year-old renal transplant recipient w...

Disseminated Cryptococcosis presenting as cellulitis in a renal transplant recipient.

Cellulitis is an unusual presentation of cryptococcal infection in renal allograft recipients. In such patients, disseminated cryptococcal infection can result in significant morbidity and mortality. Patients are often treated with antibiotics before a definitive diagnosis is made, delaying appropriate therapy. We describe the case of a 43-year-old post renal transplant recipient presenting wit...

Disseminated cryptococcosis presenting as cellulitis with necrotizing vasculitis.

Patients with disseminated cryptococcosis infrequently present with cutaneous involvement. Skin lesions, when present, are usually multiple and polymorphous in appearance. Cellulitis caused by Cryptococcus neoformans is rare, and necrotizing vasculitis associated with cryptococcal vascular invasion has not to our knowledge been reported. We report here a case of disseminated cryptococcosis in a...

Primary cutaneous cryptococcosis in a renal transplant recipient: case report.

We report a kidney transplant recipient with severe skin- and soft-tissue infection mimicking necrotising fasciitis. Patient failed to respond to empirical antibiotic therapy for presumed bacterial cellulitis. Culture of aspirate from the wound and tissue samples revealed Cryptococcus neoformans. No signs of systemic cryptococcal infection were found. After antifungal treatment and surgical int...

Necrotizing fasciitis as the initial presentation of disseminated infection with fluconazole-resistant Cryptococcus neoformans

INTRODUCTION Cryptococcus neoformans is an encapsulated budding yeast that is a common cause of opportunistic infections, rarely giving rise to cellulitis, vasculitis or fasciitis. Necrotizing fasciitis caused by C. neoformans is a rare but serious problem in post-transplant immunosuppression. CASE PRESENTATION We report a case of cryptococcal necrotizing fasciitis in the left adductor longus...
